  • All Operators must file an AOGC Form 1 - ORGANIZATION REPORT that is to be mailed to the El Dorado Commission office (address at top of page)
  • The Form 1 may require information secured from
    Secretary of State, State of Arkansas
    256 State Capitol Building
    Little Rock, AR 72201
  • Please furnish the Commission 's El Dorado office with a copy of the Certificate issued by the Secretary of State's office

If you are permitting a new well you must file the following in the El Dorado Commission office:
